AFFIDAVIT OF LOSS

I,FATIMA NIDUELAN DE JUAN, of legal age, Filipino, and a resident

of Alaminos, Madalag, Aklan,after having been duly sworn in accordance with
law, hereby depose and say:

1. That I am the legal beneficiary/claimant of the late VICTORIANA N.

LAURENIO;

2. That the late Salvador Mabasa has been issued an owner’s duplicate
copy of Transfer Certificate of Title (TCT) with number T-21523 by the
Registry of Deeds of Aklan;

That,however, the abovementioned copy of Certificate of Title was lost

whenour house was flooded because of the insurgence of Typhoon Frank in
2008, washing away almost all our documents including the said title;

That despite diligent efforts to locate the same, it could no longer be

found;

That I am executing this affidavit for the filing of my petition for the
reissuance of the above-mentioned owner’s duplicate copy of Certificate of Title;
and

That, furthermore, I am executing the same to attest to the truth of the

foregoing facts and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 14th
