AFFIDAVIT OF DISCREPANCY

I, GINA D. DEFENSOR, of legal age, Filipino citizen and a resident of Bacan

Banga, Aklan Philippines, having been duly sworn to in accordance with law,
hereby depose and state:

1. That I am the mother of IZZABELLE D. DEFENSOR and the wife of JOSE

Y. DEFENSOR JR.;

2. That the “JR.” of my husband has not been reflected in the Affidavit
of Acknowledgment/Admission of Paternity appearing in my daughter’s
Certificate of Live Birth; therefore, it appears to be “JOSE Y. DEFENSOR”;

3. That however, it is “JOSE Y. DEFENSOR JR.” that has been reflected in

my husband’s Certificate of Live Birth and in our Certificate of Marriage;

4. That I am executing this affidavit to explain the discrepancy

between my daughter’s Certificate of Live Birth and my husband’s Certificate of
Live Birth together with our Certificate of Marriage to prove that “JOSE Y.
DEFENSOR” and “JOSE Y. DEFENSOR JR.” is the same person.

IN WITNESS WHEREOF, I have hereunto set my hand this 11th day of January
2018 at Kalibo, Aklan, Philippines.
