AFFIDAVIT OF DISCREPANCY

I, EDDI DELA CRUZ TULIO, of legal age, Filipino citizen and a resident of
Linayasan, Altavas, Aklan, Philippines, having been duly sworn to in accordance with law,
hereby depose and state:

1. That ever since I was born and up to the present, I have been using “EDDI DELA
CRUZ TULIO” as my full name in all my private and public dealings;

2. That however in my Certificate of Live Birth issued by the Philippine Statistics

Authority, my name appears therein as “KENNEDY DELA CRUZ TULIO”;

3. That “EDDI DELA CRUZ TULIO” and “KENNEDY DELA CRUZ TULIO”
refer to one and the same person, and that no other person or persons will be
prejudiced by this declaration; and

4. That I am executing this affidavit to attest to the truth hereof and to explain the
discrepancy between my Certificate of Live Birth and Police Clearance, and other
documents with regards my full name.
