AFFIDAVIT OF DISCREPANCY

I, MARCELINO TUYOR BALAGAT, married, of legal age, Filipino citizen, and a resident
of Brgy. 14, San Gregorio, San Nicolas, Ilocos Norte, Philippines, after having been duly
sworn to in accordance with law hereby depose and states that:

1. My true and complete name as appearing in my Certificate of Live Birth issued by

the Philippine Statistics Authority (PSA) is MARCELINO TUYOR BALAGAT;

2. And, that ever since, I have been using the name MARCELINO TUYOR BALAGAT in
all of my transactions either public or private;

3. Vitally, however, a perusal of my Driver’s License issued by the Land

Transportation Office (LTO) bearing entry no. A10-00-007502 as well in my
Philippine National ID bearing entry no. 3715-6976-8402-7387 show that my name
as recorded and appearing therein is MARCELINO TUYON BALAGAT;

4. That my middle name as appearing in my said Driver’s License as well as in my

Philippine National ID, which is TUYON, is contrary to my true and correct middle
name, which is TUYOR;

5. Vitally, the said discrepancy in my middle name was done by mere inadvertence
and was never intended to mislead or deceive any person, office or agency;

6. I am executing this affidavit to attest that the names MARCELINO TUYON

BALAGAT and MARCELINO TUYOR BALAGAT refer to one and the same person;
and

7. Finally, I am executing this affidavit to attest to the truth of the foregoing and for
whatever intents and purposes it may lawfully serve.

IN WITNESS WHEREOF, I have hereunto set our hands this 20th day of January 2024 at
San Nicolas, Ilocos Norte, Philippines.
