SECURITY AND INVESTIGATION SERVICES INC. With postal address at 398 A. Mabini
St. Brgy. 21 Caloocan City, Philippines the registrant, its Officers and owners or principal
stockholders has not been operating or previously operating as a contractor under different
business name or entity.

That the said firms has not been charged with any labor case in the Department of Labor
and Employment and in the National Labor Relations Commission, nor having pending
cases with the same;

That I, am further executing this affidavit to affirm the truth of my foregoing statements
and for whatever legal purpose it may serve best.
