THE DUPLICATES OF OFFICIAL RECEIPTS

Dear Ms. MARIA:

This refers to the indorsement of Juan, Acting City Treasurer dated January 16,
2023. The indorsement is composed of documents and records relative to the
investigation conducted by the City Treasurer’s Office (CTO) on your non-remittance of
collections in the amount of Nineteen Thousand and Forty Pesos (₱19,040.00), and
alterations in the duplicates of official receipts in your custody.
Under the Civil Service rules, failure to remit public funds, and falsification of
public documents are grounds for the administrative case of Serious Dishonesty,
Grave Misconduct, and/ or Conduct Prejudicial to the Best interest of Service.
Under the directive of the City Mayor, this Office shall conduct a preliminary
investigation pursuant to Rule 4 of the 2017 Rules on Administrative Cases in the
Civil Service (RACCS). Thus, you are hereby directed to show cause and file a
comment/explanation/counter affidavit, within five (5) days from receipt of this letter
as to why you should not be held administratively liable.
Your failure to submit a comment/explanation/counter affidavit shall be
considered a waiver thereof and the preliminary investigation may be completed even
without the comment/explanation/counter affidavit.
