    Dear Ms. Maria:

    This is to inform you that this office shall conduct a preliminary investigation
    pursuant to Rule 4 of the 2017 Rules on Administrative Cases in the Civil Service
    (RACCS) in response to a Reklamong Salaysay (Complaint Affidavit) dated November 3,
    2022 filed at this office by a certain Mr. Juan Dela Cruz. for GRAVE MISCONDUCT,
    DISHONESTY, AND CONDUCT PREJUDICIAL TO THE BEST INTEREST OF THE
    SERVICE.

    In view thereof, you are hereby requested to submit your counter-affidavit or


    comment in writing and under oath within FIVE (5) DAYS from receipt hereof. Copy of
    the complaint affidavit is hereto attached for your reference.

    Your failure to submit a comment/explanation/counter affidavit shall be


    considered a waiver thereof and the preliminary investigation may be completed even
    without the comment/explanation/counter affidavit.

    Please give this matter your preferential attention.
