DEED OF UNDERTAKING AND ATTESTATION

I, ___________________________ of legal age, Filipino and residing at

________________________________, after having been duly sworn to in accordance with
law, do hereby undertake:

1. That I am being hired by LBP Service Corporation (LBPSC for brevity) as

______________ for assignment to Ericsson Resources;
2. That I am being assigned to Ericsson Resources for a period of ____ (___) months;
3. That by virtue of my assignment to Ericsson Resources, including any and all
extension/s of my assignment thereto, I hereby PROMISE, UNDERTAKE, and
OBLIGATE myself, under pain of criminal and civil liability, not to disclose nor
furnish any unauthorized person, partnership, firm or corporation any information that
may be acquired and/or possessed by me in the course of my rendition of services as
an incident to the Contract of Services between Ericsson Resources and LBPSC;
4. That I shall not solicit, engage or be employed with a Company/corporation who is a
direct competitor of Ericsson Resources even after my employment thereto for a
period of twenty-four (24) months;
5. That I shall not enter into an employment contract or any other agreement for the
provision of services with any person or entity in competition with Ericsson
Resources during my employment and within twenty-four (24) months from my last
date of employment thereto;
6. That should I leave without prior written consent and approval from both Ericsson
Resources and LBPSC without respect to his/her assignment period mention under
section 2 hereof (except due to serious illness or death in the family) I may be
penalize at the rate of 20% of my remaining salary for my unexpired period of
assignment.
7. That should I violate the stipulations stated under Section 4 & 5 hereof, I may
likewise be held criminally and civilly liable for such actions and even be penalize
thereon; and,
8. That this undertaking is being freely and voluntarily executed by me and I am aware
of the implications/consequence of this deed of undertaking.
