I, JESSICA Z. SANCHEZ of legal age residing at Blk 02, Lot 10, Everlasting Street, Barangay Sauyo, Quezon
City, after having been sworn hereby deposed and say:

1 – That I am the owner of the house Blk 02, Lot 10 at the above mentioned address with an estimated
area of 20 square meters.

2 – That the said structure is made of light and heavy materials and is constructed on the lot where I
have established possessory rights.

3 – That I declare the said residential house for reassessment purpose and hereby request the City
Assessor to issue the corresponding tax declaration in my favor.

4 – That I am executing this affidavit to attest to the truth of the foregoing, and for whatever legal
purpose it may serve.
