RELEASE, WAIVER AND QUITCLAIM

I, ______________________________________ of legal age, Filipino, and a resident of

_____________________________________________________________ after having been sworn in accordance with law, do hereby declare that I
acknowledge the lawful cessation of my employment with (hereinafter the “Company”) effective ______________. I also acknowledge receipt of
the amount of ______________________________________________________________________ representing all the monetary benefits due me
under the law and Company policy as a result of my previous employment with the Company. In consideration of such payment –

1. I release, remise and forever discharge the Company, its successors-in-interest, stockholders, officers, directors, agents or employees from
any legal action and claims for sum of money, damages, and any and all demands whatsoever, which in law or in equity I ever had, now have, or
which I, my heirs, successors, and assigns hereinafter may have upon or by reason of any matter, cause or things whatsoever, up to the time of these
presents, the intention hereof being completely and absolutely to release the Company, its successors-in-interest, stockholders, officers, directors,
agents or employees from all liabilities arising wholly, partially or directly from my employment therewith.

2. I will not, at any time and in any manner whatsoever, directly or indirectly, disclose to any person or entity, any information of any kind
relating to the business of the Company, its manner of operations, plans or data of any kind.

3. I further manifest that I will not file any case of whatever nature against the Company including its successors-in-interest, stockholders,
officers, directors, agents or employees in relation to any matter related to my employment with the Company.

4. I acknowledge that I have received all amounts that are now, or in the future, may be due me from the Company. I also acknowledge that
during the entire period of my employment with the Company, I received and was paid all compensation, benefits and privileges to which I was
entitled under all laws and policies of said Company by reason of such employment.

5. I finally declare that I have read and understood this document and the consequences contained therein which was further explained to me
in the dialect that I am most familiar with and the Release, Waiver and Quitclaim hereby given are made willingly and voluntarily and with full
knowledge of my rights under the law, and full acceptance of the grounds of termination or cessation of employment as true, lawful, and justified.
