SWORN STATEMENT

I, ________________, Filipino, of legal age and a resident of Lahug, Cebu City and is currently
the designated Chief Executive Officer of Asia, Inc, a corporation duly registered under the laws of the
Republic of the Philippines with principal address at Cebu City, Philippines hereby depose state that:

1. In compliance with Section 4 of SEC Memorandum Circular No. 18, the company has adopted
the Code of Ethics for the Asia, Inc Collections Unit, stating among others that:
“Chapter III
Section 8-
All employee handling the collection whether, in-house collectors or not, shall disclose
his/her full name or true identity to the borrower.”

2. That the subject procedure has been adopted as part of our operations procedure in dealing with
the various clients of the company.

3. Further, attached herein as Annex A is the full text of the Code of Ethics for Asia Inc. Collections
Unit specifically incorporating the above-mentioned provision.

I hereby certify that this Sworn Statement with its Annex are executed to attest to the truth of the
foregoing and for whatever legal purposes it may serve.
