New Washington, Aklan after having been duly sworn in accordance with law
do hereby depose and state:

1. That I am the registered owner of a certain business, with registered

name Relato Metal Works, located at Poblacion, New Washington, Aklan;

2. That I hereby declare and state that I am transferring the operation and
administration of the above-mentioned Relato Metal Works to my son
Wildy Jay Antonio Relato;

3. That I am executing this affidavit to attest to the truth of all the foregoing
statements and for whatever legal purpose it may serve.
