AFFIDAVIT OF RECOVERY I, ____________________, of legal age, Filipino, married, and a resident of ______________________________________, after having been duly sworn in accordance with law, hereby depose and say: 1. That I am the registered owner of a parcel of land situated in ________________covered under ____________________________; 2. That I filed an Affidavit of Loss dated April 27, 2006 before the Register of Deeds of Makati stating under oath that my duplicate copy of the abovementioned certificate of tile has been lost; 3. That as a consequence thereof, an annotation was made on the certificate of title on May 8, 2006 inscribed as entry number ________________; 4. That sometime on June 18, 2009, the said certificate of title which I deemed to have been lost was found in one of my cabinets while I was cleaning it; 5.
