AFFIDAVIT OF CORRECTION

I, PABLO T. RIVERA JR., of legal age, widower, Filipino, and with residence and
postal address at #33 ROXAS ST., CAUAYAN CITY, ISABELA after having been
sworn to in accordance with law do hereby state that:

1. I am the same person executing this affidavit;

2. I am the registered owner of a parcel of land with an area of THREE

HUNDRED NINETY NINE (399) square meters more or less covered by
Transfer Certificate of Title (TCT) No. 362493 which was a transfer from
Original Certificate of Title (OCT) No. P-15172 registered under the name of
my late father PABLO RIVERA SR.;

3. I instructed my son Jasper Jose M. Rivera to request for the issuance of one
title from Original Certificate of Title (OCT) No. P-15172 which was registered
then to my late father Pablo Rivera Sr.;

4. However, instead of registering the title under my name Pablo T. Rivera Jr.,
the same was mistakenly registered as Heirs of Pablo Rivera despite the fact
that I am still alive;

5. Considering the above-mentioned fact, the registered owner Heirs of Pablo

Rivera should be changed to Pablo T. Rivera Jr., in order to properly reflect
the ownership over the said parcel of land;

6. I am executing this Affidavit of Correction to attest to the truthfulness of the

foregoing, for the specific purpose of correcting the registered owner of Heirs
of Pablo River to Pablo T. Rivera Jr., and for whatever legal purpose it may
serve.
