COMES NOW, Petitioner, through the undersigned counsel,

unto this Honorable Court, most respectfully states that:

1. The undersigned counsel received an Order dated March

17, 2021 stating that the Honorable Court observed that the joinder of
two (2) petitions was improper namely to cancel second registration
of birth certificate and correction entries in the birth certificate.

2. In compliance with the aforesaid Order, the petitioner now

files this Amended Petition;

WHEREFORE, petitioner respectfully prays before the

Honorable Court to NOTE and ADMIT herein amended Petition.

COMPLIANCE

COMES NOW, Petitioner, through the undersigned counsel,

unto this Honorable Court, most respectfully states that:

1. Petitioner is required to serve a copy of the Petition on the

Office of the Solicitor General and the Office of the City or
Provincial Prosecutor, within five days from the date of its
filing and submit to the court, proof of such service within the
same period.

2. That on March 12, 2021, Petitioner filed this case with the
RTC Branch 22 – Cabagan, Isabela.

3. In compliance with the said rule, a copy of the Petition is

hereby furnished to the Office of the Solicitor General (“OSG”), The
Local Civil Registrar of San Pablo, Isabela, Local Civil Registrar of
Sto. Tomas, Isabela, Philippine Statistics Office Central Office -
Manila, Office of the Provincial Prosecutor and the Petitioner James
Limtuaco Soriano. Attached hereto is the Registry Receipt which
marked as Annex “A”, Annex “B”, Annex “C”, Annex “D”, Annex “E”
and Annex “F”.
