AFFIDAVIT OF RECOVERY OF LOST TITLE

I, ______, of legal age, Filipino citizen, married to ____, and a

resident of Quezon, San Isidro, Isabela, after having duly sworn to in
accordance with law, hereby depose and state:

1. That on February 14, 2002, I executed an Affidavit of Loss stating

that the Original Land title of a certain parcel of land registered
under the name of my husband, JC, covered by TCT No. 12345 was
lost;

2. The same affidavit was entered into the Notarial Register of ATTY.
MP as Doc. No. 75, Page No. 16, Book No. 9, Series of 2002;

3. That sometime on December 25, 2018, the abovementioned land

title was fortunately found and recovered inside our house at
Quezon, San Isidro, Isabela;

4. That I am executing this affidavit to attest to the foregoing

statements for the information of all concerned.
