I, MARILY LUARDO CABALLERO, of legal age, Filipino Citizen and a resident

of Ditucalan, Iligan City Lanao del Norte, after having duly sworn to an oath as
required by law, hereby depose and say:

1. That I am an applicant for out of –town registration of birth;

2. That I am the daughter of JUANITO A. CABALLERO and DOROTEA P.

LUARDO and born on September 19, 1970 at Aurora, Zamboanga del Sur;

3. That the facts of my birth was not promptly reported at the Office of the Civil
Registrar of Aurora,Zamboanga Del Sur;

4. That I am executing this affidavit to attest to the truth of the foregoing

allegations and to prove the facts for an Out-of-town registration of my birth
pursuant to Rule 20 Administrative Order No. 1 Series of 1993;.

3. That the birth of the said MARILY LUARDO CABALLERO was not registered at
the Local Civil Registrar’s Office, Aurora, Zamboanga Del Sur;

4. That we know the foregoing facts because we are close friends of their parents;

the said MARILY LUARDO CABALLERO with the Local Civil Registration’s
Office, Aurora, Zamboanga Del Sur and for such other legal purposes that this
