AFFIDAVIT OF CANCELLATION OF PATENT

I, ADELAIDA H. BENITEZ, of legal age, married to LEONARDO

A. BENITEZ, Filipino Citizen and a resident of 320 J. Rizal St., Tubuan 2,
Silang, Cavite, by virtue of Special Power of Attorney Represented by
ORLANDO H. BENITEZ, after having been duly sworn to in
accordance with law do hereby depose and state:

That ERNESTO HERNANDO, applicant/claimant, OF Lot 7265,

located at Sinaliw Munti, Alfonso, Cavite with an area of SIXTEEN
THOUSAND (16,000) SQUARE METERS;

That he applied for Free Patent Application in the CENRO Office,

Trece Martires City recorded as FPA (IV-4) 2711for which corresponding
Patent No. 3187 dated April 30, 1975 was issued;

That on February 24, 2016 we executed a Deed of Absolute Sale

regarding of the above-mentioned parcel of land notarized by Atty.
Magno V. Toribio Jr. docketed as Doc No. 228 Page No. 46 Book No. XXX
Series of 2016 as hereto attached the copy of the same;

That I am requesting the Registry of Deeds and DENR R-IV Trece

Martires City or other offices concerned to cancel/revoke any title
issued in the name of ERNESTO HERNANDO;

That I am executing this statement to attest to all the above

facts, and to whatever legal purpose it may serve.
