AFFIDAVIT OF CIVIL STATUS

I, ARTEMIO A. NOVESTERAS , of legal age, married, Filipino, with residence at

Lot 6 Blk 69 Woodstock Homes Navarro General Trias Cavite after having been sworn to
in accordance with law hereby depose and state that:
 
1. That I am the registered owner of one parcel of land with improvements located
at Brgy. Of Navarro Mun.of Gen.Trias Cavite under Transfer Certificate of
Title No. T-1221723 containing an area of FORTY SQUARE METERS AND
FIFTY SQ. DEC. (40.50) ;

2. That when I acquired the property my civil status is single ;

3. That on 08 May 2003 I contracted a Marriage and this present my civil status is
married herein attached the Marriage Contract as Annex “A”;

4. That I hereby formally request the Registry of Deeds to correct my civil status
from single to married to Leah Duero Novesteras in the said Transfer
Certificate of Title No. T-1221723

5. That I execute this affidavit to attest to the truth of the foregoing, and for
whatever legal purpose it may serve.
