AFFIDAVIT OF LATE REGISTRATION OF MARRIAGE

We, EDUARDO DECOSTO JR. and AQUILES ALICIA C.

ISUGAN, both of legal ages, Filipinos and residents of Poblacion East,
Moalboal, Cebu, Philippines, after having been duly sworn in accordance
with law, hereby depose and state:

1. That we are married with each other;

2. That our marriage was solemnized on September 19, 1992 at St.

Augustine Roman Catholic Church, Alcantara, Cebu, witnessed
by Alfredo Ypanto, Dominico Cabunay, Exeqiela Sabello, and
Laureana T. Rabuya;

3. That until this day we had not registered our marriage with the
Local Civil Registrar;

4. That we are executing this Affidavit to attest to the truth of the

foregoing and to support my request for registration of our
marriage with the Local Civil Registrar.
