AFFIDAVIT OF NEAREST SURVIVING KIN

I, SONNY QUIJANO TABASA, of legal age, Filipino citizen, currently

residing at Barangay 46-C Salvador St., Labangon, Cebu City, Cebu, after
having been duly sworn in accordance with law, hereby depose and state:

1. That I am the biological brother of RENIE QUIJANO TABASA,

who was born on September 15, 1966 in Cebu City and died on August 29, 2019
in Cebu City, Philippines;

2. That my brother RENIE QUIJANO TABASA have no legal

spouse and children;

3. That our parents and other siblings were dead already prior to the
death of RENIE QUIJANO TABASA;

4. Thus, I am the nearest surviving kin of RENIE QUIJANO

TABASA;

5. That I am executing this affidavit to attest to the truthfulness of

the foregoing statements and to support my claim to process and receive the
Birth Certificate of RENIE QUIJANO TABASA with the Philippine Statistics
Authority, to claim government benefits arising from his death, and for any
other legal purpose it may serve.
