I, ALLAN A. DIMALUAN, Filipino, married to DAISY DIMALUAN, of legal age,

with address at 124 Zone Ahos, 6.5 Paknaan, Mandaue City, after having been duly
sworn to in accordance with law hereby depose and say that:

1. The Certificate of my Live Birth indicates the name ALLAN A. DIMALUAN,

born on April 26, 1969, in Larina, Negros Oriental;

2. On the other hand, my other documents like school records, SSS, Philhealth,
among others, and I, myself, have been using the name spelled ALLAN A.
DEMALUAN;

3. Both names and my other documents pertain to one and the same person;

4. I am executing this affidavit to attest to the truth and for whatever legal
purposes it may serve.
