AFFIDAVIT OF UNDERTAKING

I, ROSE MARIE B. BORNALES, of legal age, Filipino and a resident of Sangi,

Toledo City, Cebu, Philippines, subscribing under oath, do hereby depose and say that:

1. That on November 29, 2021, I undertook a medical examination at

Physician’s Diagnostic Services Center, Inc;

2. That at that time, I was associated with Skyhigh Manpower Recruitment

International, Inc;

3. That I was issued a medical certificate on December 2, 2021 which would

expire on February 28, 2022;

4. That before I could commence my employment with the abovementioned

agency, I was directly hired by Mr. Ziad Riachi in Riyadh Diplomat Quarters;

5. That I am no longer pursuing employment with Skyhigh Manpower

Recruitment International, Inc;

6. That I did not undergo any major medical operations from the date of
examination up until today;

7. I execute this affidavit knowingly, voluntarily and to attest to the truth of all
foregoing and for whatever legal purpose this may serve.
