JOINT AFFIDAVIT OF LEGITIMATION

We, DEXTER JAMES H. ARIZALA and FILOMEN JOY R. LABATAN, both of
legal age, husband and wife, respectively, and presently residing at Lagundi, Oslob,
Cebu, Philippines after having been duly sworn to in accordance with law, depose and
state:
1. That we are the biological parents of KENDRA NICOLE LABATAN
ARIZALA, who was born on May 29, 2014, at Oslob, Cebu;

2. That her birth was recorded at the Municipal Civil Registrar of Oslob on June
3, 2014 under Registry No. 2014-238;

3. That at the time of her birth, we, the biological parents were not yet married;

4. That at the time when she was conceived, there was no legal impediment to
contract marriage between us, his biological parents;

5. That we subsequently contracted marriage on December 18, 2014 at the

Immaculate Conception Parish Church, Oslob, Cebu;

6. That our marriage was solemnized by Rev. Fr. Juan A. Velarde and was
recorded at the Municipal Civil Registrar of Oslob on December 22, 2014
under Registration No. 2014-142;

7. That by virtue of our subsequent marriage, the said child KENDRA NICOLE
LABATAN ARIZALA, is now legitimated by operation of law; and

8. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the legitimation
by subsequent marriage of our daughter, KENDRA NICOLE LABATAN
ARIZALA, and for all legitimate intents and purposes.

IN WITNESS WHEREOF, we have hereunto set our hands this 27 th day of

November 2021, at Oslob, Cebu, Philippines.
