AFFIDAVIT OF LOSS

I, JHON KENT IGOT, of legal age, Filipino, single and residing at Zone Sili,
Paknaan, Mandaue City, having sworn to in accordance with law, do hereby depose
and state that:

1. I am a Grade 12 student of CITE Technical Institute located in San Jose

Talamaban, Cebu City;

2. I was issued a CITE school ID with No. 196, which I always wear in going to and
from school;

3. I just found out sometime in July 2018 that lost said ID;

4. It is my belief that the said ID was lost when I was riding a jeep on my way home
from school;

5. I exerted earnest efforts to trace the said ID in my things but to no avail, and is
now considered lost beyond recovery;

6. I execute this affidavit to attest to the truth of the foregoing.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 10 th day of

November, at Mandaue City, Cebu Philippines.
