APPLICATION FOR COMMUNITY SERVICE

The accused, through undersigned counsel, unto this Honorable Court,

most respectfully applies for the imposition of community service in lieu of
service in jail, pursuant to the provisions of Republic Act No. 11362 (RA
11362) and Administrative Matter No. 20-06-14-SC (Guidelines in the
Imposition of Community Service as Penalty in Lieu of Imprisonment).

In support of this application, the accused states:

1. That he is of legal age and residing at Mohon 2, Sitio

Magtinabangay, Tisa, Cebu City. On February 17, 2023, accused pleaded and
was convicted for violation of Section 3 (a) of RA 9287 by this Honorable
Court, and was sentenced to suffer the penalty of imprisonment from thirty
(30) days to ninety (90) days;

2. That he submits that he has not previously availed of the

benefits of RA 11362 and no appeal has been perfected as of the filing of this
application;

3. That he is physically able and willing to comply with the terms

and conditions of the community service program that this court may deem
fit for him and undergo the rehabilitative counseling;

4. That the grant of his application will not depreciate nor diminish
the gravity of the crime/offense charged nor cause any undue risk that
during the period of community service, accused-applicant will commit
another crime.

PRAYER

WHEREFORE, foregoing premises considered, it is most respectfully

prayed upon this Honorable Court that this Application for Community
Service be approved. Other reliefs just and equitable in the premises are
likewise sought.
