AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, FLORDELIZA D. AGUSTIN and MERY JEN B. SIBUCO, both of legal ages,
both married, and residents of Barangays Domogok and Poblacion, Siayan, Zamboanga del
Norte, after having been sworn to an oath in accordance with the law, do hereby depose and
state: THAT-

1. We are not related by blood or by law to MARJON TUMANGGI CAMEGING;

2. We personally know the facts and circumstances surrounding the birth of the person of
MARJON TUMANGGI CAMEGING who was born to spouses WARLITO MANO
CAMEGING and NENING MANO TUMANGGI on January 04, 1995 in Domogok,
Siayan, Zamboanga del Norte;

3. That the said child was born outside of wedlock;

4. That the birth of the child was attended by Elos Manu, a traditional hilot of their own
tribe, the Subano;

5. That the said child was immunized and was baptized by Seventh Day Adventist;

6. The birth certificate of the said child was not immediately registered with the Office of
the Municipal Registrar, primarily because of the inadvertence of the parents or failure of
the person delegated to do so, hence, this Joint Affidavit;

7. That the birth certificate of the said child is not registered with any Office of the
Local/Municipal/City Civil Registrar;

8. We are executing this affidavit freely and voluntarily to attest to the foregoing narration
of facts of the birth of the child, and to support the application for delayed registration of
the same with the Office of the Municipal Civil Registrar, and for whatever legal
purposes and interest this may serve.

IN WITNESS WHEREOF, we have hereunto set my hands this October 21, 2019 in
Sindangan, Zamboanga del Norte, Philippines.
