AFFIDAVIT OF GUARDIANSHIP
I, LEANNA JAMELA B. CARANDANG, of legal age, single, Filipino citizen and a
resident with postal address at 4a-301 Seine Chateau Elysee, Brgy. Moonwalk, Parañaque Cty,
been duly sworn to in accordance with law, do hereby declare and state:

1. That I am the Grand Daughter of CONDELIA R. BANTIGUE to his daughter LANIE

CARANDANG 70 years old, who was born on March 01, 1952 at Puerto Princesa City,
Palawan;

2. That I am the one who was taking care of the above-mentioned grandparent since the
time she gets sick.

3. That due to the circumstances, I am left alone with the responsibility of the taking care of
my grandparent and living in the same roof;

4. That I am not an imbecile, insane, a vagrant or a vicious person or a habitual drunkard or

a habitual criminal, and have not abandoned, neglected or refused to support my son or
caused her to commit offenses against the law;

5. Finally, I am executing this affidavit to attest to the truthfulness of the foregoing facts and
for all legal intents and purposes this may serve well.
