Avenue, Pasig City from January 1995 to April 1996;

2. As per record of the said company, my name was written as Cepres, Reynero P.
instead of Cepres, Reynero Paglinawan;

3. I hereby affirm that Cepres Reynero P. and Cepres Reynero Paglinawan refer to one
and the same person, herein affiant;

4. The name to be retained should have been Cepres, Reynero Paglinawan;

5. Ever since, I have not asked for a certification from San Miguel Foods, Inc.;

6. I am executing this affidavit to attest the truthfulness of the foregoing and for
whatever legal purpose this may serve.
