WE, LYNMAR N. PAGLINAWAN and MIGUELITO N. MARTINQUILLA

both of legal age, Filipinos, married and residents of Tuburan, Cebu, after
having been duly sworn to in accordance with law, do hereby depose and
state that—

1. We know LONITIVA LIBRADO PISIAO because we were friends since

time immemorial;
2. Her name LONITIVA LIBRADO PISIAO is the reflecting in her Baptismal
Certificate issued by the Archdiosesan Shrine of San Antonio de Padua
of Tuburan, Cebu;
3. Recently, she discovered that her name in the Certificates of Live Birth
of her children appears to be YUNEVIVA LIBRADO PESIAO;
4. Likewise, her name registered in the DSWD Database (Pantawid
Pamilyang Pilipino Program) is UNIVIVA LIBRADO PESIAO;
5. We hereby affirm and confirm that the LONITIVA LIBRADO PISIAO,
YUNEVIVA LIBRADO PISIAO and UNIVIVA LIBRADO PESIAO refer to
one and the same person;
6. We hereby execute this affidavit to attest the truthfulness of the
foregoing and for whatever legal purpose this may serve.
