JOINT AFFIDAVIT OF EXPLANATION
WE, SANNY D. CAWALO and VIVIAN HAYDEE D. CAWALO, both of legal
age, Filipino Citizens, with residence and postal address at Guinaoang, Mankayan,
Benguet, after having been duly sworn to in accordance with law hereby depose and
state that:
1. We got married on April 11, 2011 at the Office of the Mayor, Municipality of
Mankayan, Province of Benguet as evidenced by our Certificate of Marriage
issued by the Office of the Civil Registrar of Mankayan, Benguet;
2. However in our said Certificate of Marriage particularly in the Entry: Registry
No., it is blank or there was no number that was entered;
3. The truth of such circumstances is that, right after our marriage ceremony at
the Office of the Mayor, we proceeded immediately to the reception without
having our Certificate of Marriage to be numbered by the Office of the Civil
Registrar;

4. The aforementioned circumstances are explainable and attributable to sheer

oversight and/ or honest mistake and is not tainted with any deliberate
malicious or criminal intent;
5. We are executing this affidavit to attest to the truth of our foregoing
statements, and to support our application to supplement the Registry No. of
our Certificate of Marriage;
6. We are further executing this affidavit for all other legal intents and purposes
it may serve.
IN WITNESS WHEREOF, We have hereunto set our hand this 16th day of April,
