AFFIDAVIT OF ADMISSION OF PATERNITY

I, JHON JONNY, of legal age, single, Filipino Citizen, and with residence and postal
address at 222234 Culdesac Rd., Sun Valley, Parañaque City, after having been sworn to in
accordance with law do hereby depose and state that:

1. I am the biological father of JOHN JERRY;

2. JHON JHONNY was born on 01 OCTOBER 2020;
3. The facts of the birth of JOHN JERRY were duly registered at the Office of the Civil
Registrar of Parañaque City under the following document: Certificate of Live Birth,
hereto attached as Annex A;
4. At the time of the birth of JOHN JERRY, I was not married to his mother, JENNY
JESSY;
5. I hereby acknowledge my paternity to JOHN JERRY and recognize him as my child
and legal heir;
6. I am allowing JOHN JERRYto use my surname in his Certificate of Live Birth;
7. I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever purpose it may serve.

Affiant further sayeth naught.
