JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, YUNIKKAKAY C. BINALOGA, of legal age, single, Filipino Citizen, and residing at 2422 Culdesac
Rotonda, Brgy. Sun Valley, Parañaque City and JOYLYN G. GONZALES, of legal age, single, Filipino Citizen
and residing at 4043 Culdesac Rotonda, Brgy. Sun Valley, Parañaque City, after having been sworn in
accordance with the law hereby depose and state:

1. That we personally know deceased FRANCISCO V. FORTUNO who is our long-time acquaintance in
Parañaque City;

2. That the true and accurate birthdate of the decedent is on MARCH 13, 1957, and the fact of his birth is
registered at the Office of the Local Civil Registrar General of Irosin, Sorsogon ;

3. However, as his family processing his Death Claims at Social Security System (SSS) with SS No. 03-
4805325-6, they found out that his birthdate was erroneously recorded as MARCH 17, 1956;

4. That the said discrepancy was made due to the wrong entry and confusion to his birthdate in the
aforementioned SSS Records;

5. That to remove confusion, we hereby declare that the true and accurate birthdate of FRANCISCO V.
FORTUNO is MARCH 13, 1957 that is appearing in his Certificate of Live Birth and the birthdate
MARCH 17, 1956 that is appearing in his SSS Records is referring to one and the same person;
6. That we know for a fact that the above-mentioned facts and information are true and accurate and not
intended for any illegal or unlawful purpose;

7. Finally, we are executing this affidavit to attest to the truth and to secure and testify the real identity of
FRANCISCO V. FORTUNO born on MARCH 13, 1957 for veracity of the foregoing facts and for
whatever legal purposes it may serve.
