JOINT AFFIDAVIT

We, ________________ and ________________ , both married and of legal age, having
been born in Itogon In ____ and _____ , and residents of Atok Mines, Itogon, Benguet, after first
being duly sworn to law, depose and state :

That we were both acquainted with the couple LAURENCIO C. PIANZA and
SATURNINA SOMALE, being close neighbors and intimate family friends in Atok Mines,
Itogon, Benguet;

That as such we have become privy to important events and anniversaries in their family,
such as births, graduations, marriages, deaths and the like;

That we have therefore personal knowledge of the fact that the said couple gave birth to a
female child in Itogon on March 14, 1941, was later given baptisimal name, and registered as
ERLINDA S. FIANZA;

That said ERLINDA S. FIANZA now married to LEVITICO TOQUERO, was born
during or after the World War II and that the same is not registered in the Local Civil Registry of
Itogon, Benguet;

That we are executin this affidavit to attest to the truth of all the foregoing statements
regarding the circumstances surrounding the birth of the said ERLINDA FIANZA -TOQUERO
in view of the alleged complete destruction of the birth records of the said ERLINDA FIANZA -
TOQUERO in the Local Civil Registry of Itogon during World War II, and for any legal and/or
general purposes whatsoever.

IN WITNESS WHEREOF, we have hereunto set our hands this 7th day of February,
