AFFIDAVIT OF KINSHIP
We, JIMMY ANONUEVO, JEANELITO ANONUEVO, LERMA
ANONUEVO ROLLON, of legal age, Filipino, with address at Taywanak, Alfonso
Cavite after having been sworn to in accordance with law, hereby depose and state:

1. I am the sibling of late LOLITA PEJER ANONUEVO, who died on

August 8, 1984 at Taywanak Ibaba, Alfonso, Cavite;

2. That we are the next of kin/closest surviving relatives of the

deceased LOLITA PEJER ANONUEVO, JIMMY ANONUEVO,
JEANELITO ANONUEVO since the children and no personal
representative has been appointed to process or claim any
documents of my late mother;

3. That I am making this oath for the release of any documents of my

late brother in my favor as available next of kin from any
government office/agency;

4. That I have executed this affidavit to vouch for the truth of the
foregoing and for any legal purpose it may serve.
