AFFIDAVIT OF WAIVER

I, VOTHI PHUC, of legal age, Vietnamese Citizen and a resident of Zamora,

Mangatarem, Pangasinan, after having been sworn to according to law, do hereby depose and
state:

1. That I am the aunt and legal guardian of the late HUNG QUOC VO who died on
February 10, 2015;

2. That the parents of the late HUNG QUOC VO is nowhere to be found;

3. That the remains of HUNG QUOC VO will be cremated on February 12, 2015 in
Eternal Gardens, Dagupan City and as the guardian of the said deceased, I hereby
waive any rights or claim against Eternal Gardens who will conduct the cremation;

4. That I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve.
