AFFIDAVIT OF LOSS
I, AIZEL SOMERA VALDEZ, of legal age, Filipino, with address at 183
Esperanza Ilaya, Alfonso Cavite after having been sworn to in accordance with law,
hereby depose and state, that:

1. I was a former employee of JTE Group of Companies Inc., and was

issued a company ID with no. JTE 1576;

2. However upon checking my files, I found out just recently that said
company ID was lost and cannot be found;

3. I exerted diligent effort to find the said company ID but the same
could no longer be found; hence, considered forever lost;

4. I am executing this affidavit not for any unlawful purpose but for
any valid and legal purpose only.

IN TRUTH WHEREOF, I have hereunto affixed my signature this 20 th day of
