AFFIDAVIT OF KINSHIP
I, YOLANDA M. CREDO, of legal age, Filipino and with address at Purok
3, Bilog, Alfonso Cavite, after having been sworn to in accordance with law, hereby
depose and state:

1. That I am the granddaughter of the late PEDRO MOJICA, who

died on October 8, 1978 in Alfonso Cavite ;

2. That I am the next of kin and one of the closest surviving relative of
my said grandfather and no personal representative has been
appointed to process or claim any documents of my late
grandfather;

3. That I am making this oath for the release of any documents of my

late grandfather in my favor as available next of kin from any
government office/agency;

4. That I have executed this affidavit to vouch for the truth of the
foregoing and for any legal purpose it may serve.
