AFFIDAVIT OF EXPLANATION

I, FLORINIA C. ELGAR, Filipino, of legal age, and a resident of Alley 59, Block
18, Bermai Floodway, Barangay San Andres, Cainta, Rizal, under oath, do hereby
depose and states, that:

I am the grandmother of the late JULITO A. CORPUZ (“Julito” for brevity);

My grandson succumbed to Hypoxic Ischemic Encephalopathy on January 15,

2024 in Cainta, Rizal;

We are in the course of processing the registration of his death at the Local Civil
Registrar of Cainta, Rizal;

It so happened, however, some of our relatives who are processing the

registration had erroneously written a number (“98-240”) on Registry Number portion of
the Municipal Form No. 103 (Certificate of Death);

I hereby attest that the writing on the above-portion was due to honest mistake
and was unintentional on our part. It was neither done to defraud anyone nor delay legal
process;

I hereby certify that the statements made herein were true and correct to the best
of my knowledge and belief.
