AFFIDAVIT OF DISCREPANCY

I, CONRADO E. RIMA, Filipino, of legal age, and currently residing at No. 08 Swimming
Street, Barangay San Juan, Cainta, Rizal, after having been duly sworn to in accordance with
law do hereby depose and say:

1. I was born of Hacobe R. Elen and Benegno A. Rima on OCTOBER 12, 1962 in
Caramoan, Camarines Sur;

2. I customarily use the birthdate OCTOBER 12, 1962 in all my personal and public
records;

3. It so happened, however, in my Voter’s Identification card issued by COMELEC-Cainta,

Rizal, I found out that my date of birth was erroneously registered as OCTOBER 12,
1960, when in truth and in fact should have been registered as OCTOBER 12, 1962;

4. I hereby attest that Conrado E. Rima with birth date of October 12, 1960 and Conrado E.
Rima with birth date of October 12, 1962, refer to one and the same person and that
OCTOBER 12, 1962 is my true and correct date of birth;

5. That I am executing this affidavit to attest to the veracity of the foregoing facts and for
whatever legal purpose it may serve under the law.

IN WITNESS WHEREOF, I have hereunto set my hand this 18th day of December 2023 in
