JOINT AFFIDAVIT OF COHABITATION

We, ERIC T. DELA CRUZ and CATHLEEN KAY B. TATEL, Filipinos,

both of legal age, and both residents of No. 238 San Jose Street, Barangay
Commonwealth, Quezon City, under oath do hereby depose and state, that:

We are executing this affidavit for exemption of marriage license under

Article 34 of the Family Code of the Philippines;

We hereby attest that we have been living together as common-law

husband and wife for thirteen (13) years;

Consequent to our cohabitation, we begot our daughter RICCALEEN T.

DELA CRUZ on July 7, 2012 in Quezon City, Metro Manila. A copy of her
Certificate of Live Birth is hereto attached as Annex “A” and made an integral
part hereof;

Moreover, we do not have any legal impediments to marry one another;

This AFFIDAVIT is being executed to attest to the veracity of the foregoing

statements in order to establish our cohabitation for our application for exemption
of the requirement of a marriage license, purposes not contrary to law, morals,
good custom and public policy.
