We, ALFREDO R. LUCIANO, of legal age, Filipino Citizen, and residing at 32 Hunters ROTC Ave.
Brgy. San Juan, Cainta, Rizal, and TERESITA G. LUCIANO, of legal age, Filipino Citizen , residing at 32
Hunters ROTC Ave. Brgy. San Juan, Cainta, Rizal, after having been duly sworn to in accordance with law,
hereby depose and say;

1. That we personally know ANICETO DAGUMAN ERIA, being our close friend and neighbor in
Cainta, Rizal, and being the daughter JULIET ENDITA ERIA;

2. That we know for a fact that ANICETO DAGUMAN ERIA who is a member of the SOCIAL
SECURITY SYSTEM (SSS) died on October 11, 2023 in Cainta, Rizal;

3. That upon presentation of the necessary documents, with the SOCIAL SECURITY SYSTEM
(SSS) personnel questioned the daughter of the Deceased about the discrepancy/error of the
middle name of the deceased;

a. ANICETO ERIA - as per SSS Records;

b. ANICETO DAGUMAN ERIA – as per Batismal Certificate and Death Certificate and
Negative Certificate of Birth;

4. That the real and correct name of the deceased was ANICETO DAGUMAN ERIA as registered
and record in his Batismal Certificate and Certificate of Death;

5. That we excuting this Affidavit to attest to the truth of all the foregoing and to establish the
fact that the name of the deceased ANICETO DAGUMAN ERIA and ANICETO ERIA
belong/refer to one and the same person and for all legal intents and purposes that this may
serve.
