JOINT AFFIDAVIT

We, LEONOR B. LOZADA and EROLLEN B. BANTILING, both of legal age

and Filipino, both residents of Cadiz City, Negros Occidental, Philippines, after having
been duly sworn to in accordance with law do hereby depose and say;

That we both personally MA. SOCORRO L. IBAÑEZ, being the legitimate spouse
of CARLOS N. IBANEZ, who is also a Filipino Citizen and resident of Cadiz
City,Negros Occidental;

That we have personally known the person of MA. SOCORRO L. IBAÑEZ for a

long period of time having been her long-time friend and mentor;

That we know for a fact that the person by the name of “SOCORRO L. IBAÑEZ”

appearing in the GSIS Information for Membership and the PSA Marriage Contract and
the name “MA. SOCORRO L. IBAÑEZ” as appearing in the Funeral Application Form
and LCR Death Certificate, refer to one and the same person and that her
true and correct name is MA. SOCORRO L. IBAÑEZ;

That we are not in any way related to her either by consanguinity or by affinity
neither do we have any pecuniary interest in this matter actual or expected;

As such, we execute this Affidavit or attest to the truth of the foregoing

facts and for whatever legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, we have hereunto affixed our hands this 9 th day of
