JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

That we, REYSHELEE GUINANG, of legal age, Filipino, single and a resident of
Looc, Sibulan, Negros Oriental, and JASON BRENZ ALFRED OLLENA, of legal age, single,
Filipino and a resident of Poblacion, Sibulan, Negros Oriental, under oath, do hereby depose and
manifest:

That we personally know the person of the late “CRESENCIANO IDO DOROY”, and
that recently we came to know that his name as indicated in his Philhealth Records/data was
“CRESENCIO IDO DOROY”, while in his Birth Certificate is “CRESENCIANO DOROY”,
and we know for a fact that the person of “CRESENCIO IDO DOROY” as appearing in his
Philhealth Records/data, “CRESENCIANO DOROY” in his Birth Certificate, and the name
“CRESENCIANO IDO DOROY”, REFERS TO ONE AND THE SAME PERSON, the
latter being the correct and real name;

Perhaps, the incorrect entries were due to honest mistake and inadvertence. That we
know of these facts considering that we are acquiantances and friends of him, and that neither of
us is related with him either by affinity or consanguinity and that we are executing this joint
affidavit freely and voluntarily in order to attest to the veracity of the facts above-stated and for
whatever legal purposed this may serve best;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 15 th day of

September 2022, at Sibulan, Negros Oriental.
