AFFIDAVIT OF TWO DISINTERESTED PERSONS

We ANGELITA B. FERNANDO., of legal age, Filipino, married and residents of

Agbannawag Rizal Nueva Ecija, and JULIETA B. DEONA, of legal age, married,
Filipino and resident of B03 L21 Rafael St. Sav 7 Pulang Lupa Dos after having been
duly sworn to in accordance with law, hereby depose and state:

1. That we personally knew LOLITA PEROCHO CAGOLCOL who appeared to

be LOLITA PEROCHO CAGOLCOL in her one of SSS ACCOUNT NUMBER
3354029527 with Birthdate: MARCH 17, 1957 AND NOLITA PEROCHO
CAGOLCOL in her second SSS ACCOUNT NUMBER 34-1296173 with
Birthdate: SEPTEMBER 17, 1957 being our neighbors and close acquaintance;

2. We declare confirm and affirm that the stated NAME and BIRTHDATE of LOLITA
PEROCHO CAGOLCOL stated in her TWO SSS ACCOUNT NUMBER are the
same person stated herein.

3. That it was mistakenly encoded her name and birthdate without further checking
on the aforementioned requirements submitted.

4. That her real name was LOLITA PEROCHO CAGOLCOL with BIRTHDATE OF
SEPTEMBER 17, 1957 and has been using the said name and birthdate in all
her documents and transactions

5. That we are not related by affinity or consanguinity to the above mentioned

person; and

6. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts without any promise or reward or any other monetary
consideration.
