AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MANOLO CALDEJON JR , of legal ages, Filipino, and residents of B13 L9 BAMBOO
HEIGHTS PASONG KAWAYAN 2 GENERAL TRIAS CITY CAVITE, and JENNYROSE CALDEJON, of
legal age, Filipino and resident of B13 L9 BAMBOO HEIGHTS PASONG KAWAYAN 2 GENERAL
TRIAS CITY CAVITE after having been duly sworn to in accordance with law, hereby depose and
state:

1. That we personally knew FREDERICK JOSE MENDOZA BAUTISTA and JINNAH MERIDA
MALAPITAN , being our neighbors and close acquaintance;

2. We declare confirm and affirm that their name stated in the PSA Birth Certificate of
ARJAY MALAPITAN who appeared to be their son that we attest that their real name
was FREDERICK JOSE MENDOZA BAUTISTA instead of FREDERICK MENDOZA BAUTISTA
and JINNAH MERIDA MALAPITAN instead of GINA MERIDA MALAPITAN.

3. That it was mistakenly encoded the name without further checking on the
aforementioned application for Birth Certificate under the supervision of AMANG
RODRIGUEZ MEDICAL CENTER MARIKINA CITY.

4. That they has been using the said name’s in all their documents and transactions please
see attached copy of Birth Certificate of JINNAH M. MALAPITAN and FREDERICK JOSE M.
BAUTISTA, which are marked as ANNEX “A”, series to form part hereof.

5. That we are not related by affinity or consanguinity to the above mentioned person; and

6. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts without any promise or reward or any other monetary
consideration.
