MANIFESTATION

Private Complainant, thru the undersigned

representative, and unto this Honorable Court, most
respectfully state that:

1. That on January 31, 2019, Mr. Ulyses Sanchez (duly

authorized representative of Luzon Development
Bank) transacted in this Honorable Court to get the
payment on the above stated respondent amounting
to Five Thousand Pesos (Php. 5,000.00). Attached
herewith is the copy of Secretary Certificate which is
marked as Annex “A”.

2. In view of, the private complainant would like to

authorize Mr. Ulyses G. Sanchez duly representative
of the bank to get the payment of the respondent
Ms. Corazon Pablo.

3. In addition, the complainant would like to suggest

that the payment be made at exactly 3:00PM in the
afternoon, every end of the month (30 TH or 31st) until
the obligation is done base on the stipulated
agreement.
 4. This motion is filed in good faith and solely for the
foregoing reason.

In view of the foregoing, it is most respectfully prayed that an

order be issued inserting the name of Mr. Ulyses G. Sanchez of
those personnel who are authorized in getting the money
amounting to Php. 5,000.00 from Ms. Corazon Pablo.
