AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, GINA P. ASIDILLO, of legal age, Filipino, and residents of B03 L14 Mindoro St. Deca
Homes Sahud Ulan Tanza Cavite, and BEVERLY F. TALACTAC, of legal age, married, Filipino and
resident of Brgy. Sahud Ulan Tanza Cavite after having been duly sworn to in accordance with
law, hereby depose and state:

1. That we personally knew ANALYN GAYAGAYA HAGUISAN who appeared to be ANALLEN

M. GAYAGAYA on her own Birth Certificate, being our neighbors and close acquaintance;

2. We declare confirm and affirm that her first name was ANALYN instead of ANALLEN. that
this is to attest that the name stated herein belongs to the same person.

3. That it was mistakenly encoded the name without further checking on the
aforementioned registration or document. please see attached copy of his Identification
cards to prove the veracity of claim herein, which is marked as ANNEX “A-series”, to form
part hereof.

4. That she has been using the said name in all her documents and transactions

5. That we are not related by affinity or consanguinity to the above mentioned person; and

6. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts without any promise or reward or any other monetary
consideration.
