AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MARILYN CADAG MATEO, of legal age, Filipino, and residents of 441 F Dela Cruz St.l
Caridad Cavite City, and JOSEPH ANTHONY AYALDE GRADO, of legal age, Filipino and resident
of 369 Salcedo St. Noveleta Cavite after having been duly sworn to in accordance with law,
hereby depose and state:

1. That we personally knew CARL VINCENT AYALDE TIRONA , being our neighbors and close
acquaintance;

2. We declare confirm and affirm that his name stated in all records in United States of
America is CARL TIRONA instead of CARL VINCENT AYALDE TIRONA. that this is to attest
that the name stated herein belongs to the same person.

3. That it was mistakenly encoded the name without further checking on the
aforementioned registration or document. please see attached copy of his Identification
cards to prove the veracity of claim herein, which is marked as ANNEX “A”, to form part
hereof.

4. That he has been using the said name in all his documents and transactions

5. That we are not related by affinity or consanguinity to the above mentioned person; and

6. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts without any promise or reward or any other monetary
consideration.
