AFFIDAVIT OF LOSS & DISCREPANCY

I, ANI NIMPHA C. CASTRO, Filipino, of legal age, married, and a resident of Poblacion,
Minuyan, Norzagaray, Bulacan, after having been duly sworn in accordance with law, hereby depose
and say:
1. That I am the owner of a Postal I.D. issued by the Philippine Postal Corporation (Nueva
Ecija) bearing the name “Ani Nimpha C. Castro ”;
2. That in the above-mentioned Postal I.D., the year of my birth was recorded to be “1985”
3. That in my Certificate of Live Birth issued by the Philippine Statistics Authority, my TRUE
and CORRECT year of birth is 1984.
4. That I lost said Postal I.D. and that I exerted all efforts to locate the same but I could no
longer find it and it is considered lost for all legal purposes;
5. That said Postal I.D. was not confiscated by any officer of the law by reason of any
violation of any law, ordinance, presidential decree and neither was it offered as collateral
for any loan;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and to reconcile
and correct the apparent discrepancy of my year of birth in my LOST Postal Identification
Card (PID) as well as to support my application for a new Postal I.D. and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 1 st day of December 2020
